We only collect the information you provide. We do not collect information from you through your IP address, computer settings or most-recently visited URL. We do not collect any personally identifiable information about you unless you voluntarily submit such information to us by filling out a form, survey or registration form.

In general, we will only use the information you provide to us for the purpose for which such information was provided. We may also use this information to deliver to you information about Choice, promotional materials, surveys, and newsletters. Your information may also be used to contact you when necessary. Users may opt-out of receiving future mailings from Company by following the instructions set forth in the opt-out section below.

We do not provide your personal information to third parties without your consent.

Cookies are small pieces of information stored on your hard drive, not on our site. We do not currently use cookies.

Services Provided by Choice

Choice helps you narrow down retirement and senior care options by working directly with you. We discuss your care requirements, preferred area, price level, desired social interaction, preferences and concerns.

With the information you provide, we will call providers to inquire about vacancies and to make sure they can accommodate your needs and preferences. We will not provide the home with your contact information but will give them your name and a basic outline of your needs and preferences to help narrow down your available options. It is the provider’s responsibility to conduct a full assessment of care needs.

We review our most recent information regarding complaints filed against homes. We do not verify or warranty the accuracy of the information provided to the county or the accuracy of the information provided from the county to us. We do not guarantee that each of the homes on the list we provide you do not have a complaint registered against them. You are free to inquire when we last updated the complaint information, if the homes we suggest do have complaints, and what those complaints are.

Once we narrow down the options, we will contact you and provide you with the list of home names, the contact person and the address and phone number of the home. You will have the option to schedule tours on your own or have us set up the tours for you. We can accompany you on tours or provide you with a tour booklet to tour at your own leisure. We cannot guarantee that we will be available to accompany you on the tours on your time line.

We do not guarantee that any room or apartment will continue to be available. We cannot put holds or reservations on rooms for you. Choice does not negotiate price; rental rates and costs are to be discussed between you and the provider.

We are to remain a neutral party. We are not owned or affiliated with any specific provider or community provider. We will not endorse any provider but provide options to you.

Collection of Fees

There is no charge to the consumer for our referral services. We collect a fee from the homes and communities if you choose one that we suggested. This fee is based on a standard contract and absolutely should not affect your rent or care costs. If you find out it has impacted your cost in any way, we ask that you call us so that we may investigate and remove the provider from our list.

Privacy Information

Choice will not sell our information to any third parties except as already described herein. We will retain your contact information and may from time to time send you information, surveys or thank you notes, or other correspondence. If at any time you do not wish to receive correspondence from us, please let us know and we will cease further communication. Choice does not use Cookies or other programs to gather data from your computer.

Limits of our Service

We do not sponsor or endorse any third parties, their product, service, promotion, website, or their information provided on this site. You are responsible to make the decision regarding housing and care and shall use your own judgment in doing so. If you need advice beyond our area of expertise we can refer you to a third party. If you contact a third party for services, our relationship does not extend to the services provided by the third party and you agree to hold us harmless of any negligent or unlawful act committed by the third party.

We do not act as Case Managers. We do not assess, plan, implement, coordinate, monitor, or evaluate the options and services required to meet your health care needs. We are not responsible for any external links supplied on our Web site the accuracy of the information, the proper working of the Web sites, or any interaction you have with that third party vendor.

Your use of our Services

You are solely responsible for maintaining the accuracy of all information you provide and the transmission of the services or products you buy or sell through this site. You acknowledge and agree to the terms of our privacy policy as set forth on this site.

By submitting or supplying information you are requesting Choice assist you with finding senior housing. We ask that you inform us if you are using other similar services to find housing. Either party can terminate service with notice to the other party.

Prohibited Conduct

You agree not to supply information that is unlawful or may be used for unlawful purposes. This includes anything that is harassing, libelous, invasive of another’s privacy, harmful, obscene, tortuous, infringes on another’s intellectual or real property rights, or is otherwise objectionable.

You agree to not commit fraud by representing yourself as someone else, use false information including but not limited to using a false email addresses. You are not to submit any information that is copyrighted, protected by trade secret or otherwise subject to or appropriates a third parties’ rights. Choice shall be held harmless for any of the above conduct on your part as against any third party or yourself.

You shall not use this site to send unsolicited emails, advertisements, mail or fax.

Notify Choice immediately if you believe someone used your email to obtain information or breach this site.

You are granted a limited license to use this site and the information, materials and data supplied on this site. This shall be for your own use or use within your family and shall not be sold or sublicensed to any other party. Your limited license is revocable at any time.

You agree to not interfere or make an attempt to interfere with this site or the special workings of this site. You agree to personal jurisdiction as well as venue in the State and Federal Courts sitting in Multnomah County, Oregon and waive any objection to such jurisdiction or venue. Any claims must be brought within one year after the cause of action arises or such claim is barred.

Mediation Requirement

The parties shall attempt, promptly and in good faith to resolve any dispute or conflict from use of the site or services. If the parties are not able to resolve the dispute or conflict within a reasonable time either party may submit the controversy or dispute to mediation. Under these terms mediation must be sought before either part may file suit in court. If the parties cannot resolve the dispute or conflict through mediation then the parties shall be free to pursue a case in the applicable court within Multnomah County, Oregon.

Feedback

Any feedback or referrals provided to Choice are deemed to be non-confidential and Choice shall be free to use the information for any lawful reason. THESE TERMS OF USE ARE SUBJECT TO CHANGE WITHOUT NOTICE.
